Study Design
Study Design: - Multicentre, prospective observational cohort study
•Study Population: - Adult patients (≥18 years) undergoing primary liver transplantation.
•Preoperative Assessment: Clinical evaluation, routine laboratory parameters and CPET. Scores to be computed as follows:
- MELDNa: MELD + sodium adjustment per UNOS formula.
- MELD 3.0: Includes bilirubin, INR, creatinine, albumin and gender factor.
- ALBI: (log₁₀ bilirubin × 0.66) + (albumin × −0.0852).
- CPET: Peak VO₂, anaerobic threshold, and VE/VCO₂ slope.
Primary Outcome
- To compare the predictive performance of MELDNa, MELD 3.0, ALBI score and CPET parameters for perioperative morbidity and mortality in liver transplant recipients.
Secondary Outcome
- To assess correlations between preoperative scores/CPET parameters and postoperative complications (ClavienDindo grade ≥III).
- To evaluate predictive value for ICU/hospital stay, graft dysfunction and survival.
- To develop a combined risk model integrating biochemical and functional predictors.
- To examine subgroup variations (living vs deceased donor, ACLF vs CLD, etiology) and assess additional models such as CLIFC ACLF or PALBI for incremental value.
